Now that you have a personalized health action plan, it’s time to start tracking your progress! The MY Health Tracker will help you remember your diet and fitness goals and help you stay committed to your plan. An interactive checklist makes it easy to remember what to do and simply check it off. Reminders can be set to help you remember when and how often to eat certain foods or take supplements. Progress charts will let you see how you’re doing over time. You can even rate your day and see how your ratings relate to your progress.

Sometimes, it’s hard to remember which foods fall into which categories. With the MY Health Tracker you can drill down into each action on your checklist to find out more about that food group and how best to prepare it. You can also remind yourself which health risks that food is associated with. Similarly with exercise, you can drill down to learn more about the recommended exercise and what health targets it relates to.

With any new health plan or goal, it’s always hard to stay committed. The MY Health Tracker is designed to be a quick and simple way to stay engaged with your Action Plan and start seeing the health benefits. Start whenever you’re ready to make lifestyle changes that stick. We want you to stay healthy!
